Complete the application for your account and affiliate website business. Within the application, type in 2 paragraphs on what your website will be about and why you started your business. Once that’s done, you can relax because we will support you through the rest. As you work, we will be your Point-of-contact so we can coordinate the launch of your website. Submit your application/request for enrollment on your official letterhead and submit your 501(C)(3 ) Certification. We cannot donate any monies to your organization until we receive these documents. If you are an Association, Club, Family, Group, or Business, we are required by law to issue you a 1099 form if you do not have the proper non-profit certification. Therefore, we need your organization’s leader, family lead, or business owner to submit an IRS W-9 Form.
